Transitional Care Manager to coordinate your care from admission to discharge

Access to greenhouse, library, and Fireside Cafe

Clinical Capabilities

Medical director and physician extenders on-site 5 days a week

Stroke and neurological event recovery

Labs, doppler studies, and X-rays completed on-site

Individualized service plan designed by care team

IV medication administration

Wound care collaboration through Chestnut Hill Hospital
